fa چاقی و اوبِزوژن‌های محیطی به‌عنوانِ عواملِ هم‌افزای بروز و پیشرفتِ سرطان‌های وابسته به هورمون Obesity and Environmental Obesogens as Synergistic Factors in the Development and Progression of Hormone-Dependent Cancers فیزیولوژی Physiology مقاله پژوهشی Research Article <div style="border-bottom:solid #002465 1.5pt; border-top:none; border-left:solid #002465 1.5pt; border-right:none; padding:0cm 4.0pt 1.0pt 0cm">زمینه و هدف: بدخیمی‌های وابسته به هورمون، از جمله سرطان‌های پستان، پروستات، آندومتر، تخمدان و تیروئید از شایع‌ترین سرطان‌ها در سراسرِ جهان هستند. چاقی با القای هیپرلپتینمی، مقاومت به لپتین، کاهشِ سطحِ آدیپونکتین، مقاومت به انسولین و تسهیلِ تبدیلِ آندروژن‌ها به استروژن‌ها در بافتِ چربی، سرطان‌زاییِ هورمونی را تسریع می‌کند. عواملِ چاقی‌زای-محیطی (مانند بیسفنول A، فتالات‌ها، دیوکسین‌ها و PM2.5) از طریقِ اختلال در گیرنده‌های هورمونِ جنسی، فعال‌سازی مسیرِ AhR و القای استرسِ اکسیداتیو، اثرهای هم‌افزایی با چاقی دارند و در نتیجه مقاومت به لپتین را تشدید می‌کنند. استرسِ مزمنِ شهری این اختلال‌ها را بیشتر تقویت می‌کند. هدفِ مطالعه، روشن کردنِ مکانیسم‌های مشترکِ این سه عامل در ایجادِ سرطان‌های وابسته به هورمون است. مواد و روش‌ها: این بررسیِ دامنه‌دار طبقِ دستورالعملِ PRISMA-ScR انجام شد. جست‌وجوی سیستماتیک در پایگاه‌های دادۀ PubMed، Scopus، Web of Science و SID از ژانویۀ ۲۰۱۹ تا نوامبرِ ۲۰۲۵ با استفاده از کلماتِ کلیدیِ چاقی، لپتین، آدیپونکتین، اوبِزوژن‌ها، BPA، فتالات‌ها، PM2.5 و سرطانِ وابسته به هورمون انجام شد. از ۱۲۸۴ مقالۀ شناسایی‌شده، ۶۹ مقاله معیارهای ورود را داشتند و در سنتزِ روایی گنجانده شدند. نتایج: چاقی، خطرِ ابتلا به این سرطان‌ها را تا هفت برابر افزایش می‌دهد. عواملِ چاقی‌زای-محیطی اثرهای هم‌افزاییِ قوی با چاقی نشان می‌دهند و استرسِ مزمنِ شهری این اثرها را از طریقِ محورِ HPA تقویت می‌کند.</div> <p style="border-color: currentcolor currentcolor rgb(0, 36, 101) rgb(0, 36, 101); border-style: none none solid solid; border-width: medium medium 1.5pt 1.5pt; padding: 0cm 4pt 1pt 0cm; text-align: justify;">نتیجه‌گیری: تعاملِ بینِ چاقی و اوبِزوژن‌های محیطی، محورِ بیماری‌زای مشترکی را در سرطان‌های مرتبط با هورمون تشکیل می‌دهد. کاهشِ وزن و به حداقل رساندنِ قرارگرفتن در معرضِ آلاینده‌های محیطی می‌تواند در جلوگیری از پیشرفتِ بیماری و بهبودِ پیش‌آگهی مؤثر باشد. <div style="text-align: justify;">Background and Aim: Hormone-dependent malignancies, including cancers of the breast, prostate, endometrium, ovary, and thyroid, rank among the most prevalent cancers worldwide. Obesity significantly accelerates hormonal carcinogenesis by inducing hyperleptinemia and leptin resistance, reducing adiponectin levels, promoting insulin resistance, and enhancing the conversion of androgens to estrogens in adipose tissue. Environmental obesogens such as bisphenol A, phthalates, dioxins, and PM2.5 exert synergistic effects with obesity through disruption of sex hormone receptors, activation of the AhR signaling pathway, and induction of oxidative stress, thereby aggravating leptin resistance. Chronic urban stress further amplifies these disruptions via the hypothalamic–pituitary–adrenal (HPA) axis. This review aims to elucidate the shared mechanisms by which obesity, environmental obesogens, and chronic stress contribute to the development of hormone-dependent cancers. Material and methods: This scoping review was conducted according to PRISMA-ScR guidelines. A systematic search was performed in PubMed, Scopus, Web of Science, and SID databases from January 2019 to November 2025 using relevant keywords including obesity, leptin, adiponectin, obesogens, endocrine-disrupting chemicals, BPA, phthalates, PM2.5, and hormone-dependent cancer. Of 1,284 identified articles, 72 studies satisfied the inclusion criteria and were incorporated into the narrative synthesis. Results: The findings show that obesity can increase the risk of hormone-dependent cancers up to sevenfold. Environmental obesogens synergize strongly with obesity by disrupting sex hormone receptors, activating the AhR pathway, and inducing oxidative stress, which worsens leptin resistance. Chronic urban stress further potentiates these alterations along the HPA axis. Conclusion: The complex interaction between obesity and environmental obesogens constitutes a common pathogenic axis driving hormone-related cancers.
